| Or send an e-mail. When requesting an estimate via e-mail, please provide your name and Kronos ID number. The estimate will be mailed to your home address as stored in PeopleSoft. | |

| Have you recently moved and need to submit a change of address? | |

Change of Address Form - Check the box indicating "New Home Address" and complete the form in its entirety. |

| Do you need to make changes to your direct deposit information? | |
|
Direct Deposit Form - Complete the top portion of this form and have your financial institution complete the bottom portion. |

| Do you need to cancel your direct deposit? | |

Cancel Direct Deposit Form - Check the box indicating "Cancel Direct Deposit", initial and complete the form in its entirety. |

| Do you want to make changes to your federal withholding? | |

Tax Withholding Form - Complete the first page of this form in its entirety. |
